You are about to start the process to become a Crisis Care Member of Perfect Pet WOOF Club:

By joining you are confirming your acknowledgement and agreement of our Terms and Conditions of Membership, including the commitment to a 12 month payment schedule. You must keep up with the monthly payments to retain your Membership and all included benefits. Payments will be taken on the same date each month, commencing exactly one month from the date you join.
We will be in touch within 72 working hours (Monday to Friday only) of your enrolment to discuss your chosen package and register your pet(s), and your welcome pack will be ready for you to collect at your first appointment. If you would like this to be posted a postal fee of £2.50 will apply.

Your WOOF Club Health & Wellness Crisis Care membership will entitle you to the following:

Eligibility to apply for our 24/7 Emergency Veterinary Care Fund and be awarded up to 80% of the costs associated with emergency care anywhere in the UK
Access to our 24/7 pet health advice line
 
Emergency Procedure
 
If you experience an urgent / emergency situation with your pet, please contact us as soon as possible on 0330 043 4550 where one of our clinical team will carry out a triage assessment over the phone. You will be advised of the appropriate course of action.
If you are advised to visit us or another veterinary practice, please follow this advice within the recommended timeframe. If you don't and your pets condition deteriorates, you may not be eligible for a grant award.
When your pet has been taken in for assessment or treatment, please ask for an itemised estimate or invoice and email it to: accounts@animalmagicfamily.co.uk
We will assess this invoice within 1 hour of receipt and inform you of the grant award that you will receive.
The awarded grant will be sent to your specified bank account within 28 days of confirmation.
 
What Our Grants Cover
 
Urgent or emergency veterinary care for your pet which is classed as life threatening, or will become life threatening if not treated within 72 hours
Immediate (within 3 days) follow up care / veterinary check
 
What Our Grants Do Not Cover
 
Incidents which occur within 28 days from the date of joining
Travel expenses to our Centre or veterinary practice of your own choosing
The management of chronic conditions (such as Diabetes or heart failure)
Post operative management (such as x-rays, ultrasound or further testing)
 
Eligibility
 
If you are on a low income, you will be eligible to be awarded up to 80% of the total cost. 
If you are not considered to be on a low income, you will be eligible to receive up to 60% of the total cost.
In exceptional circumstances, you may be able to apply for a further grant to cover the remainder of the invoice.

Your Monthly Payment
 
Your monthly payment is taken as a contribution to our Emergency Care Fund for Member pets who need urgent veterinary care. Please be aware that this accumulative Fund exists to help all Crisis Care Members and is not solely for your own pet(s) care. This Fund should be considered a safety net, not insurance.
